Website Forms: Capture Leads From Your Existing Site Forms
Keep the forms you already have and let Alivo catch every submission.
Website Forms is an Alivo Capture tool that listens for submissions on contact forms you already have on your website. You do not need to rebuild your forms or change your page design. A small embed script sits on your site, reads the form fields when someone hits submit, and sends the lead into Alivo. This guide covers what it does, how to set it up, and what happens after a form is submitted.
What Website Forms Does
When a visitor submits a form on your site, Alivo:
- Creates or updates a contact using the name, phone, email, and other fields from the form.
- Tags the lead source so you can see it came from Website Forms in reporting.
- Starts a conversation with the agent you linked to this lead source.
- Sends the agent's start text so the prospect gets a reply in under a minute (as long as the agent is Active).
- Captures tracking info like UTM parameters and Google click IDs when they are on the page URL.
The script uses a background send method so the visitor's form still submits normally on your site. They should not notice any delay.
1. Create the Lead Source
Open Lead Sources
Go to Settings → Integrations → Lead Sources and click Add Lead Source. Choose Website Forms (marked Alivo Capture).
Name It and Link an Agent
Give the lead source a clear name (for example, "Main Website Contact Form"). Select the agent that should handle follow-up for these leads. The agent must be Active for automatic texting to run.
Copy the Embed Code
After you save, Alivo shows an embed code snippet. Copy it and paste it into your website's HTML just before the closing </body> tag. You only need one copy of the script per site, even if you have multiple forms.
2. Map Your Form Fields
Alivo needs to know which form fields hold the phone number, name, email, and so on.
- Submit a test form on your live site after the embed code is installed.
- Go back to the lead source settings and open the Field Mapping section.
- Match each captured field to the right Alivo contact field.
Alivo also keeps a recent submissions log in the lead source settings so you can confirm captures are coming through without opening your browser's developer tools.
Heads up: Alivo skips sensitive fields like passwords and credit card numbers automatically. You do not need to worry about those being captured.
3. What Happens When Someone Submits
Here is the full flow:
- Visitor submits your form. The Alivo script reads the field values and sends them to Alivo.
- Alivo creates the lead. If that phone number already exists, Alivo updates the contact instead of creating a duplicate.
- Your agent sends the start text. The first message comes from the agent's configured start text.
- Follow-ups continue. If the prospect does not reply or book, the agent's follow-up sequence keeps running.

When to Use Website Forms
Website Forms is the right choice when you already invested in your website and do not want to swap out your forms. It works well with WordPress, Elementor, and most standard HTML forms. If you want a visible widget or a booking flow with calendar abandonment follow-up, look at the Chat Widget, Pop-up Widget, or Booking Widget instead.
Website Forms is the simplest way to make sure every form submission on your site turns into a real conversation, not a row in a spreadsheet nobody checks until tomorrow.
